Long form sent. What next?

Hello. I sent back my long form a few weeks ago. Wanted to know if I should receive next in the mail. I don't even want to call the office. Should I receive a confirmation with the name of a contact or what should I expect next? Thanks so much

If you sent back form 454 and the medical release, usually the next thing you can expect to come in the mail is form SSA-3733-BK which is the AFR (Adult Function Report) for you to fill out and they sometimes send one to your 3rd party contact if you listed one. Form 3373 will have some VERY similar questions as form 454. Kind of annoying. I literally put the exact same responses word for word if the question was the same to save myself some mental effort.

Form 3733 should come from your state's DDS. Mine had the caseworker's name and contact info and the barcoded cover page on top. YMMV as far as how long it takes them to mail out form 3733. Took about 3 weeks for me between when I mailed 454 back and when I received 3733 in the mail. And I think I've heard of one person who got their benefits continued without ever receiving form 3733 so maybe that's a possibility too though not the norm?

The adult function form being sent then in my opinion lends some credence to the belief that they are just looking for you to make one wrong remark on your adult function form. Why.... Because why are they sending out another form (adf) that asks basically the same exact questions that the 454 asked you. What's the point? To me it just seems like they are looking for a reason to say "ha gotcha" you answered that differently than you did on the 454. Just thinking out loud.
