Question 5 SSA-455 General or Detailed...

Knowing how limited line space is on SSA-455 Question 5n (Reason for visit) and some put detailed reasons, others general. MY last 3 visits were Psychotherapy (With two providers, one is general therapy with a licensed therapist, the other is with a psychotherapist for Anxiety and Sleep Disorder) No psychiatrist visit in the most recent 3 visit period due to the transition.

Will putting down "Psychotherapy" be good enough or does it need more detail for why  Will Too general cause a possible flag by the computer? (I'm a Low with a low score in the 40s according to the scanline information)

I read over on "How to Get ON" blog )(Sad the creator passed away - alot of helpful information that made thing easier for me back when I first applied and my first long form CDR Review) that there's no right answer and some people are detailed with conditions and other are more general in responses. U checked The Billing CP codes and they are "Pstchotherapy".

Personally, I think you could put Psychotherapy OR you could put Anxiety (or whatever the main disorder is you are being treated for/were approved based on). You could even put Psychotherapy for one and put Anxiety (or whatever) for the other two visits. Honestly, I don't think it matters. You can even put down ANXIETY PSYCHOTH or ANXIETY THERAPY. (I used ANXIETY THERAPY for two of the spaces I had for my appointments with my psychologist. If my main disorder was a longer word than ANXIETY, I would have probably just fit as much of the disorder name as I could in the boxes.)

FWIW, I think I've seen it recommended more often to put the condition you are being treated for (like anxiety) vs. the actual treatment (like psychotherapy).

New York State put out a guide on filling out CDR Reviews, short and long forms, several years ago which stated that you should put what you were approved for in the reason for visit boxes, fill out as much as what fit in the boxes and then stop, not to continue in the comments section. It also said do not put down visits for anything besides what you were approved for. 

Videonitekatt I don't know why you are hung up on putting down the three most recent visits for what you were approved for. If you went to different medical specialties for what you were approved for, you can go back further in the time period to put that provider visit in. in your case the psychiatrist.  For instance i may see a pulmonary doctor 3 times before i see my neurologist again, but I don't just put down the three pulmonary visits, i put down two pulmonary and one neurologist. my approved conditions are pulmonary and neurological.

I concur. For me, I see my psychiatrist and psychologist for the same conditions, so for me it didn't make much of a difference since all the appointments were generally for "anxiety". But IMO you can pick and choose if you want all of your conditions represented, assuming you have appointments in the last year for all your specialists. I've never heard of this hurting you later on in the rare case you get chosen for a long form.

I have not gotten my first CDR yet, but thanks to everyone here, i agree with their help for you. I suffer from 3 mental disorders, but only 2 were approved during ALJ hearing. If it helps for me I'm going to do this:

DEPRESSION
DEPRESSION
ANXIETY THERAPY

My BPD doesn't count right now.

Good luck on your CDR, I hope things turn out great.

I just want to do this right so there's no issues when the computer reads the form. I'm stressed out over this, and related issues as it is. I want to make sure I use the correct terms and terminology.

But it does say "Most Recent Visits" in order of recent to oldest. I just want to follow the instructions to the letter.

If I've not mentioned, I'm a Low/0047 - so there's still that 5% chance it could get kicked to a long form full CDR so that's Why I'm making sure everything is right, proper and "by the book."

Perhaps an over-abundance of caution, yes, but you never know.
