New York State put out a guide on filling out CDR Reviews, short and long forms, several years ago which stated that you should put what you were approved for in the reason for visit boxes, fill out as much as what fit in the boxes and then stop, not to continue in the comments section. It also said do not put down visits for anything besides what you were approved for.
Videonitekatt I don't know why you are hung up on putting down the three most recent visits for what you were approved for. If you went to different medical specialties for what you were approved for, you can go back further in the time period to put that provider visit in. in your case the psychiatrist. For instance i may see a pulmonary doctor 3 times before i see my neurologist again, but I don't just put down the three pulmonary visits, i put down two pulmonary and one neurologist. my approved conditions are pulmonary and neurological.
