I guess i feel very scared that I am going to write something down that is going to hurt me from getting approved. I am supposed to send this back by wed. I havent done it yet. I almost dont want to send it back. And just let my medical records do the talking any thoughts.

You Just need to do it , but be truthful and make sure you tell them the same thing you told them in the past , I'm sure you made this out before. make a copy for yourself ( I make a copy of everything )otherwise you may forget what you said pryor . Thats when you have a problem with SSA when you change your story from the last time you made it out  and that may cost you to lose . Don't be afraid  just do it. you'll feel better for it.  :good-luck:

I think my attorney asked me the questions, and helped me fill it out.  They sent it in with my inital application.  I'll have to ask them for a copy.  The attorney did send me a pain form, and a form about work and what the dr thought I was capable of.  Good idea about keeping copies of everything.

Hello Patrick,

I think there is a double post going here . . . anyhow, yes, complete the form for it is a document to be utilized by SS and any HONEST info that you provide will be a positive for you.  Do not procrastinate, do not stop, pass GO and collect the money!  Whoops, I think I was just playing a gameboard!  Seriously, there are a lot of awesome folks on these boards, and a particular few that provide awesome advice.  They are the folks in which I read their forums and was able to move forward with my own matter.  We are all in the same boat, we have all been in the same boat, and we all want to make it ashore . . . fill the forms out!

Patrick,

If the form is going to be late you need to call your case worker as you do not want them to think you are non compliant and make a decision without all the evidence you can provide.

They will likely call you and then send a follow up letter however it will add a lot of time to the processing of your case.  It is really easiest just to fill it out as it will take a lot more out of you to worry about it and have someone trying to contact you.

So what happens after you fill out the Function Report and Drug Report? I just got it today.

I applied on March 23rd, my caseworker mailed in my paperwork the 29th and SSA Indianapolis sent my forms out the 3rd. Is this good timing or just typical of them?

I'm really nervous about applying but I cannot see myself going back to work in my mental condition.

Hi I sent mine in I freaked out about it about it for weeks. Dont waste as much time doing it as i did. I was hyperventilating when it went in the mail because I was afraid screwed something up. Mine was a week late i called my examiner she is actually quite pleasant. Lets hope she approves my claim. Good luck.

DDS uses those reports to help determine the severity of your impairment (step 2 in the sequential evaluation process). The Adult Function Report is an SSA form and can be found on our Forms page in fillable/saveable format. You can download it to your computer and fill in as draft format. Go back add, edit, delete information untl you are comfortable with your answers. That will hopefully take some of the pressure off of handwriting the answers and hoping to get it right the first time. And, our fillable forms remove the lines for handwriting, replacing them with boxes where a lot more information can be typed in.

The Drug Report is an optional in-house form from your state DDS, so that isn't available on our forms page.
