Adult Function Report - Good Advice

Hi everyone, i recieved Funtion report adult and function report third party and a work history report yesterday, im lost .

can anyone please guide me to where on the site i can find some some logic on how to answer these questions the right way. is it normal to be sent all of these at the same time im in shock. i only have a few days to get them back . any help will help thanks.

The best advice i can give you is be honest, but at the same time dont  list any hobbies that'll look like a job skill, like for example filling out spread sheets,  also if you say that you cut your own grass be sure to say if it puts you off of your feet, to where you can barely move. When they read your papers they will see what you write, and will only take it how you write it since they do not see you until the hearing really. This is only my opinion, because i've been there and done that anyone else is free to give theirs

I did see somewhere wher another member went thru all the questions and explained the best way to answer them using your own personal conditions but im unable to find it now . ITS ALOT OF FORMS FOR ME TO GET DONE AS FAST AS THEY WANT ME TOO . my third party has alot of problems concentrating and shes the only person i have that can do it for me . im so lost and i dont want to put anything down that will kill my app . Anyone that can help me would be great Thanks all your the best

 I feel like my biggest problem is how to word everything on the Adult Function report and the third party report . i know that it must be done right
or i risk saying something wrong and my third party is affraid of the same thing. im sooooooooo lost! how do others get thru this maze i would like to hear from others that have mastered these forms Thanks everyone

My best advice is to be descriptive.  Don't say "I can't lift things"  say instead "I am unable to lift over x amount of pounds due to the herniated disk in my back. Lifting anything over x pounds causes my back to spasm and I must take x medication.  This medication causes me to be sleepy, confused, dizzy and unable to focus." 

I think the more descriptive the better.  Just diagnosing yourself is meaningless to social security.  It's the same idea on the third party. The more you can illustrate the toll your disability has taken on your life the better.

  If i only had someone that can concentrate, i write down everything you and others give me tips on and when i go to put it on paper i get confused , i hate these forms and i have a rep and they tell me they will help with the work history and adult function but not the third party function report although they are almost the same .

It is in your reps best interest to view the third party form prior to it being sent in.  While they won't assist with it, I am sure they would be happy to view it prior to sending and offer advice on any changes.

I dont know what you are looking for ?Are you asking us what you should say on the RFC?If so nobody can answer those question but you.You know how these conditions affect you AND WHAT YOU LIMITATIONS ARE.You have allready been given great advice just be honest in your response to each question. Ill give you a idea how i filled mine out if that will help you ( I suffer from Vertigo,bad back,shoulder and neck so no bending,twisting,squatting.Question 12.)personal care - explain how your illnesses injures or conditions affect your ability to dress . I answered it this way I can put on my pants by sitting down and pulling them up and my wife puts on my socks,my shoes are already tied so i just slip my feet in them.Bathing I take a shower and my wife washes from my knees down and she sits in the bathroom while I shower to make sure i dont slip and fall from vertigo...ect and ect.......explain how you do those things and if limitations on why or how you do those things with the limitations.does that make sense to you.As I stated before we cant tell ytou what to say you and you alone know your limitations and how they affect your daily living.hope this helped

What im asking is how to word alot of these answers .

after you have told me how you answered yours that helps me alot.

i seen somewhere where a person went thru the entire Adult function report  saying on each question instead of saying X say this when you explain your daily functions using your conditions . but as luck would have it i cant find it now .
