Hi Marci thank you for your reply , The info im lookig for i may have found on another site . the one you posted for me to look at you gave me yesterday . i appriciate all your help your the best , i wish some one would post one of theirs here with all there info marked out everyone i talk to needs help with the adult function and third party function . PLEASE POST IF YOU HAVE ONE THAT WORKED FOR YOU IT HELPS US ALL .

The problem is that they are very individual.  What is meaningful for one means nothing to another.

I think the most important thing about filling out the Adult Function Report is to list a disability as described in the Blue Book.  Make it easy for the reviewer to approve you with facts.  Don't rely on gaining sympathy.  The success of a DDS reviewer is determined by the number of cases returned by SSA during Quality Reviews.  The fewer the better.  Since DDS doesn't know which cases will be selected for QR, they treat all cases the same way.  Even if SSA returns a case with questions, it doesn't look good for the DDS reviewer.  Try to make it as clear cut as you can.  List the disability and note how it prevents you from working.  That's what DDS focuses on.  The rest is often skimmed.

Thanks for the great info lost2me and yorker, it really help me and others to get the Adult function report and third party report done the right way . its my hope that everyone working together will help  more of us get full favorable decisions. this is a great site and the nice people that run it are the best , im hopeful that i dont make mistakes on these forms .

Hi everyone i have a few questions about Adult function report versus third party function report . my adult function report is done and sent in and what puzzles me is on the third party form that my friend is doing is it supposed to mirror the adult function report . please advise on theses questions please.


section( C ) of the form ask about abilities

it doesnt have much room to write do the want to know about which causes pain or do they want to know how it limits my ability to do these things such as standing and lifting . the problem i have is that with my bad back and knee and shoulders and hands and hearing i ended up checking almost all of them . so now i think how will i describe all of these limitations HELP PLEASE

I didn't even try to put all my explanations on the form itself. I had 4 different attachments and then just put - see atachment a....b.....c....etc

The third party report is basically the same. It should be from the third parties perspective. If he/she doesnt know....they should put that.

I did ask for a short extension with filling mine out. My son has two packets to fill out about me as well.
I'm VERY nervous about it because (as you all know from reading my posts  ) I tend to ramble when anxious.

If I get stuck I may call on support. everyone here has been so super.

Crap I think those third party reports suck lemons....haha
My daughter and the one friend i have left should get those again. I think maybe my daughters
 may help the most.

On the other hand, my husband and the same friend filled them out the first go around, and if ya'll
remember the judge said they didnt hold much weight since they would tend to agree with me,
so WTF,(sorry my past life i was a female pirate or sailor) why do they even ask for them, they are mandatory i hear if you file for mental impairments.

I just filled mine out and sent it in about a week ago (the 24th of May, I was accidentally late doing and she gave me an extention). But now after reading this I'm worried that I may have wrote myself out of SSI Its hard for me to write so I write sloppy and then they barely gave enough room on the form for me to put everything so I put as much as I could (even writing down the page sometimes). My mom got the 3rd party form and said that she put a ton of details in there so not sure if that could be my saving grace or not but I kind of feel bummed about it now.

I asked for an extension also because the forms are long and tedious. I did not even get a third party report, but I did get a work history report to fill out along with a fatigue and pain supplement. Strange because the first time I got the third party report when I was denied at the ALJ hearing, and he even referred to it. My adult function report is very complete and includes over 22 pages of attachments.

I never knew I could add extra attachments to it That would've helped. Its already painful to write a lot and trying to write small made it even harder.
